Steps to cancel:
1. go to http://fitnessconnectionusaforms.com/
2. Enter User name: workout Password: 1allDay40 (not usually required anymore)
3. Fill out online cancellation request form. Use the client email address.
4. The cancellation verification from Fitness Connection will be sent directly to the client.
5. You may then fulfill the client case- no need to send Successfully cancelled email.
6. When you submit the form it asks for the username and password again. Then it submits. It almost always show the confirmation as a blank form but it really goes through correctly. They have a 30 day cancellation policy.

2. Enter User name: workout Password: 1allDay40 (authentication not currently required)
3. Fill out online cancellation request form. When you submit the form it asks for the username and password again. Then it submits. It almost always show the confirmation as a blank form but it really goes through correctly. They have a 30 day cancellation policy. It generally takes 5 days for the cancel request to show on the gym level, you can verify the cancellation at that time.
